The following patient rights and responsibilities have been written in accordance with the Patient Rights Regulation No. 30657 dated 16.01.2019.

PATIENT RIGHTS

1- Right to Benefit from Health Services

  • The patient has the right to benefit from health services in accordance with his needs, including activities to promote healthy living and preventive health services within the framework of the principles of Justice and Equity.

2- Right to Information and Request Information

  • Sick; He/she has the right to request information verbally or in writing about how and under what conditions he/she can benefit from health services.

3- The Right to Choose and Change the Health Institution and Personnel

  • Sick; He/she has the right to choose the health institution and organization and to benefit from the health services provided in the health institution he/she chooses, provided that the procedures and conditions stipulated by the legislation to which he/she is subject are complied with.

4- Respect for Privacy and the Right to Request It

  • Sick; has the right to request that medical evaluations regarding his health condition be carried out confidentially.

5- Right to Request Determination of Priority Order

  • The patient has the right to request that his priority be determined objectively and based on medical criteria. Elderly and disabled patients have the right to priority when receiving health care.

6- Right to Diagnosis, Treatment and Care in accordance with Medical Requirements

  • The patient has the right to request diagnosis, treatment and care in accordance with the requirements of modern medical knowledge and technology.

7- Right to Medical Attention

  • Health Personnel gives the patient the medical attention required by the situation. Even if it is not possible to save the patient’s life or protect his health, it is imperative to try to reduce or relieve his suffering.

8- Right to Review Medical Records and Request Correction of Records

  • Sick; may examine and obtain a copy of the files and records containing information regarding his/her health status, directly or through his/her attorney or legal representative. These records can only be viewed by those directly involved in the patient’s treatment.

9- Right to Consent

  • Sick; The patient has the right to participate in the treatment by giving informed consent before any surgery or treatment.

10- Right to Refuse and Stop Treatment

  • Sick; Except for legal and compulsory cases, the patient has the right to refuse and request the cessation of the treatment planned or being applied to him, with the responsibility of the negative consequences that may arise.

11- The Right to Ensure Security

  • Sick; Everyone has the right to expect and want to be safe in healthcare institutions and organizations.

12- The Right to Fulfill Religious Obligations

  • A religious officer who is in line with their religious beliefs is invited upon request, in order to give religious advice to patients and support them spiritually, provided that they do not cause disruptions in the institution’s services, do not disturb others, and do not interfere in any way with the medical treatment organized and carried out by the staff. 13- The Right to Respect for Human Values

  • Sick; He/she has the right to receive health care in health institutions and organizations that befits human dignity, where all necessary hygienic conditions are met, and where noise and all other disturbing factors are eliminated.

14- Right to Have Visitors/Companions

  • Sick; Depending on the working procedures and principles of the health institution and organization, the patient has the right to have a companion or visitor, to the extent required by the patient’s health condition, as permitted by the legislation and facilities of the institution, upon the approval of the doctor responsible for the treatment.

15- The Right to Take Protective Measures

  • Sick; has the right to receive (see) appropriate service (treatment) to protect against disease.

16- Right to Quality Standards

Every individual has the right to benefit from high quality healthcare services within the scope of compliance with strict standards and specifications.

17- The Right to Avoid Unnecessary Pain/Suffering and Distress

  • Every individual has the right to be protected from pain and suffering to the extent possible at all stages of his or her illness.

18- Right to Personal Treatment

  • Sick; They have the right to be directed to diagnosis and treatment programs according to their personal needs.

19- Right to End of Life Care

  • The right of the patient to receive treatment that will comfort him in the terminal period when healing is not possible and keep his quality of life at the highest level is respected.

20- Right to Application, Complaint and Lawsuit

  • The patient and those who are related to the patient have the right to make all kinds of applications, complaints and lawsuits within the framework of the legislation in case of violation of patient rights.

OBJECTIVES OF PATIENT RIGHTS

As Private Bağcılar Oral and Dental Health Polyclinic, it is to determine and notify the rights and responsibilities of patients and their relatives who apply for examination and treatment during the period they receive service in the institution.

RESPONSIBILITIES OF THE PATIENT AND HIS FAMILY

1-   General Responsibilities

  • Our patient must follow the treatment plan and recommendations recommended by the doctor responsible for his treatment for a healthy life.

2-   Social Security Status

  • Sick; must report changes in their health, social security and personal information in a timely manner.

3-   Informing Healthcare Professionals

  • Sick; It must provide accurate and complete information about the medical history to date, current complaints, treatments applied, medications used and changes in their condition.

4-   Compliance with Institutional Rules

  • Sick; He/she must act in accordance with the rules of the health institution to which he/she applies and comply with the practices to be followed.

  • The patient; It is desired to cooperate with healthcare professionals throughout the treatment and care.
  • Sick; If they benefit from a health institution that provides service by appointment, they are asked to comply with the date and time of the appointment and any changes must be reported to the relevant office.
  • Sick; The institution must respect the rights of its employees, other patients and visitors.

5-   Compliance with Recommendations Regarding Treatment

  • Sick; The patient should carefully listen to the recommendations regarding the medications he uses in his treatment and ask about the parts he does not understand.
  • The patient; If there is a situation of not being able to comply with the recommendations regarding treatment, the patient should report this to the healthcare professional.
  • Sick; It is necessary to indicate whether he/she understood the health care and post-discharge care plan correctly as expected.
  • Sick; He/she is responsible for any problems that may arise due to refusing the treatment or not complying with the recommended treatment plan.

6- Responsibility to Pay Health Care Expenses

  • Sick; At the application stage to the health institution, the person must provide information about how and by which institution or organization the health expenses will be covered. The patient is obliged to pay the amount required for the applications performed for diagnosis and treatment within the period determined by the institution.

7-   Responsibility to Report Communicable Disease

  • Patients with a suspected or diagnosed infectious disease are responsible for not leaving the institution unless they have permission from the authorized doctor indicating that there is no harm in leaving.

8-   Responsibility Not to Make Improper Requests

  • Our patients should not request the administration or application of any medication that is not deemed appropriate by the authorized physician and is not included in the treatment plan.

9-   Responsibility to Comply with Visitor Rules

  • Our patients are responsible for accepting their visitors within the framework of the rules set by our institution.
